The "after" photographs here are less than 2 weeks from the original surgery. This Los Angeles area woman came to my Pasadena office for consultation about weakness of her lower face. When we looked at her carefully, we saw that her whole jaw was probably a bit small for the rest of her face. She looked great otherwise. Now, there are complex surgeries which can correct problems like this, which involve sawing at and re-attachment of the jaw bone to itself in various places, but they carry higher risk of irregularities in the jawline and sensation changes. We suggested polymerized silicone implants, which are white, firm plastic, for her whole jawline, including the "angle of the mandible" and the chin. These were recommended because they are simple to insert and remove if they cause any problem or are undesirable cosmetically. They rarely generate complications such as infection or nerve stretching and numbness. I like to insert the chin implant through a 1/3-inch incision under the chin (it can be inserted through the mouth but has a higher chance of infection this way). The angle of the mandible implant is inserted through the lower jaw inside and goes under the jaw muscle (the masseter). This very pretty young woman now looks just fantastic, and didn't even have any significant bruising. Her lower face is now just as pretty as her upper face. The implants can't really be felt (they feel just like bone) and won't move if they are allowed to heal in place for a few weeks. There has never been a reported reaction to solid silicone in the body, no allergic reactions or other problems. Solid silicone is used in many other applications such as heart valves as well. This patient originally didn't want her photos shown on the website, but when she saw how good she looks, she told us to go ahead. She's still a little swollen at this point.
